Enrollment forms needed:
All forms are fillable online and can then be saved and printed off in advance, for your convenience.
New students only:
Please bring a copy of Baptismal certificate*
(If baptized at St. Fidelis, St. Ann or St. Boniface than certificate is not required.)
New AND returning students:
Tuition for St. Fidelis, St. Ann, & St. Boniface parishioners is $20 per student (no tuition fee for kindergarten). Tuition for individuals outside of tri-parishes is $50 per student. Financial assistance is available to families, by request.
